The Stanford Health and Lifestyle Assessment (SHALA)
is a 15 minute online survey that assesses individuals' biometric risk factors
for disease; lifestyle behaviors around physical activity, eating habits, stress
management, alcohol consumption and tobacco use; and respondents' readiness for
making a lifestyle change based on objectives put forth by the US Dept of Health
and Human Services, in a document called Healthy People 2010. for guidance in
implementing a lifestyle change.
